The Department of Health in Regional Office 6 announced that there will be a special examination for applicants in the Human Resource for Health Deployment Programs 2018 who failed to print their confirmation slips and similar reasons.
HRH Deployment Programs 2018 hires nurses, doctors, midwives, dentists, medical technologists, pharmacists, and allied health workers for deployment in grassroots level. Around 25,000 of these workers will be deployed nationwide next year.
DOH RO6 conducted the qualifying exams for Negros Occidental applicants on November 6, Aklan and Antique applicants on November 9, and Iloilo, Capiz and Guimaras on November 13.
But some applicants who went through online registration failed to print their confirmation slips which are required in admission to testing centers.
In showing consideration to health workers with such cases or other similar reasons, DOH RO6 issued the following advisory, dated November 21, 2017:
ADVISORY: All 2018 HRH Deployment Program Applicants:
Please be informed that there will be a special examination for those applicants who failed to print
their confirmation slips and other similar reasons on December 1, 2017 at the 3rd Floor, DOH Regional
Office VI, Q. Abeto St, Mandurriao, Iloilo City. First batch is scheduled at 8 AM.
All examinees area advised to bring the following on the day of exam, to wit:
• PRC ID ( other government issued IDS are allowed for other health allied professions applying for PHAIUHCI)
• Confirmation Slip/ Proof of Confirmed Application
• Black Ballpoint pen
• Long Brown Envelope that contains the following:
✓ Personal Data Sheet/CSC Form No. 212 (Download HERE) with recent 2×2 picture
✓ Photocopy of Certificate of Board Rating
✓ Certifications (Trainings and Work Experience)
✓ Photocopy of Transcript of Records
✓ Photocopy of Diploma
✓ Photocopy of VALID PRC ID (back to back)
✓ Photocopy of Tax Identification Number ID (back to back) – Non-mandatory requirement
✓ Marriage Certificate (if applicable)
✓ BARANGAY CLEARANCE
The Long Brown Envelope must be labeled using the format below:

Cadre:
Schedules per batch shall be on first come, first served basis. Applicants with multiple cadre
shall choose which of the applied cadres shall be prioritized in the application.
